Your CareLink Care Coordinator, a specialized nurse or
social worker assigned to ease your transition, will talk with
you about your stay and answer any questions you may have. |
A small group of CareLink physicians and nurse practitioners will manage your health and write orders for your treatment while you are with us. Your family doctor is welcome at any time to visit you and to discuss your care with your CareLink Physician. Your CareLink physician will also notify your family doctor in writing upon your admission and discharge, to keep you family doctor well informed of how you are doing.
If your health requires you to be evaluated by a physical, occupational, or speech therapist, you can expect to see a therapist Monday through Friday from 8 a.m. to 4:30 p.m. We want you to know ahead of time that CareLink is a non-smoking environment with no exceptions. In keeping with our mission to improve health, CareLink physicians and staff are ready and willing to assist you with the tools your need to quit smoking, should you so choose.

Before You Arrive
Our goal is to make your transition to CareLink smooth and worry-free.
Given your specific condition and the needs your physician and case manager share with us, we expect that you will be with us a mutually determined number of days. In most cases, length of stay ranges from three to six weeks. We share this with you in advance of your arrival to CareLink, as we need your commitment to this length of stay and your active participation during this time to help you achieve the best outcome.
