Before You Arrive

Our goal is to make your transition to smooth and worry-free.

Given the needs your physician and case manager share with us, we expect you will be with us a mutually determined time. In most cases from three to six weeks. We share this with you in advance, as we need your commitment and active participation to help you achieve the very best outcome.

CareLink Arrival

Your CareLink Care Coordinator, a specialized nurse or social worker assigned to ease your transition, will talk with you about your stay and answer any questions you may have.

 

A small group of CareLink physicians and nurse practitioners will manage your health and write orders for your treatment. Your family doctor is welcome to visit and discuss your care with your CareLink physician. Your CareLink physician will also notify your family doctor in writing upon your admission and discharge, to keep your family doctor well informed of how you are doing.

If your health requires you to be evaluated by a physical, occupational, or speech therapist, you can expect to see a therapist Monday through Friday from 8 a.m. to 4:30 p.m. CareLink is a non-smoking environment. In keeping with our mission to improve health, CareLink physicians and staff are ready and willing to assist you with the tools your need to quit smoking, should you so choose.
