Admission Criteria

Patients may be admitted to CareLink for the following reasons:

 

> Require daily monitoring by a physician and 24-hour nursing care

> Require special services offered by CareLink

> Medical complications have caused a functional decline

> Extended recuperation is expected

> Patient or family education is needed

> A team approach to care is required

> An appropriate discharge plan is expected.

Patients referred to CareLink will be evaluated by a medical professional to assure that they meet admission criteria. CareLink confers with the patient's personal physician to determine what is best for the patient and will recommend alternative placement if admission criteria are not met. All pre-admission screenings are free of charge. Patients may inquire about CareLink themselves or may be referred by their physician, friends or family members, social worker, case manager, allied health professional, discharge planner or insurance company.
